Part of ClickUp ticket 12 (Admin Enforcement and Reset Commands): the idp:enforce-admin-2fa half. idp:reset-2fa is not included; it depends on the shared disableTwoFactor service method from ticket 14 and will follow in a separate PR.
New idp:enforce-admin-2fa command. It reads two_factor.enforced_groups and pages the members by id in chunks of 100.
For each user with zero unused recovery codes it:
generates a batch through RecoveryCodeService::generateRecoveryCodes,
sends the new TwoFactorEnforcedMail (no codes in the mail or in the console output),
records an enrollment_changed audit event with {"source": "enforce-admin-2fa"}.
Users that already have unused codes are reported as skipped and keep the same rows.
--dry-run writes nothing and sends nothing, and prints the same processed / skipped counts a real run would.
Prints a processed / skipped / error summary. Exits non-zero if any user errored.
A failed email counts as an error for that user and is not rolled back, since rolling back would invalidate the freshly generated codes. The mail is sent synchronously (Mail::send) so the failure is visible in the counts.
SDS deviation
SDS idp-mfa.md section 3.6 says the command sets two_factor_enabled=true and two_factor_enforced_at=now(). That predates group-based enforcement in User::shouldRequire2FA(). This PR deliberately does not set two_factor_enabled, two_factor_method or two_factor_enforced_at: setting the stored flag would keep a user enrolled after they leave the group, which is self-enrollment semantics, not enforcement semantics. The SDS needs to be amended.
Things to know
The plaintext codes are never kept, so the user cannot view them later and has to regenerate from the profile Security section. The email says so, rather than promising they can "view" them as the ticket wording suggests.
generateRecoveryCodes writes its own recovery_codes_generated audit row using IPHelper::getUserIp(), which is an empty string in console. The enrollment_changed row uses 127.0.0.1. The service was not changed.
The audit method for enrollment_changed is email_otp, the only method currently supported (and the default of two_factor_method).
Test plan
tests/unit/Console/EnforceAdmin2FACommandTest.php: 5 tests (processed, skipped, users outside the group, dry-run, one error does not stop the run).
tests/EnforceAdmin2FACommandIntegrationTest.php: 3 tests against the DB (10 codes, 1 audit row with metadata, 1 email, flags unchanged, second run skips and keeps the same hashes, dry-run writes nothing).
Full phpunit suite not run, only the two files above.

Walkthrough
The change adds an Artisan command to process users in configured enforced groups. It generates recovery codes, records enrollment-change audit events, and sends enforcement emails. The command supports dry runs, skips users with unused recovery codes, and reports per-user errors.

The registered command selects users in configured groups, processes them in ascending-ID chunks, and supports dry runs. It skips users with unused recovery codes. For other users, it generates recovery codes, records an audit event, and sends an email. Unit tests cover these behaviors and per-user errors.
Newly processed users may appear to have recovery codes they cannot use, and a failed notification may not be retried. Resolve these onboarding risks before merging.

@romanetar The skip criterion needs to change with the amended SDS (ftn-docsnsklz sds/idp-mfa.md rev 2.5, §3.6): skip users that already have an enrollment_changed audit event with metadata source=enforce-admin-2fa, not users with unused recovery codes.
Today the codes are committed at line 101 before Mail::send at line 113, so a transient SMTP or audit failure leaves the user "skipped" on every later run and they are never notified. With the command no longer generating codes (see line 101), write the audit row only after the email was sent, and key the skip on that row. A re-run then retries exactly the users whose email failed. Suggested test: the mail throws on the first run and is sent on the second, with one audit row at the end.
The command no longer generates recovery codes (users get them when they enable 2FA from the profile); IRecoveryCodeService is gone from handle().
Skip criterion is now an enrollment_changed audit event with source=enforce-admin-2fa (wasAlreadyNotified()). The metadata is filtered in PHP, so no MySQL JSON_EXTRACT dependency.
Order is Mail::send first, then the audit row. If the mail throws, no row is written and the next run retries exactly that user. If the audit write fails after the mail was sent, the user gets the mail again (at-least-once); commented in the code.
Tests: new testMailFailureIsRetriedOnNextRun (mail throws on run 1, sent on run 2, one audit row at the end); the integration and unit tests were adapted to the new skip criterion. Both files pass inside idp-app.

@romanetar The command doesn't check two_factor.enabled. With the flag off, User::shouldRequire2FA() returns false for everyone, but this would still email every admin that "Two-factor authentication is now required on your account".
The SDS rollout (idp-mfa.md steps 43–46) deploys with TWO_FACTOR_ENABLED=false and only flips it right before this command, so running the steps out of order is a realistic operator mistake.
Suggested fix: at the top of handle(), return early with a warn() (and self::FAILURE) when config('two_factor.enabled', true) is false. Add a unit test for it.
handle() now returns early with a warn() and self::FAILURE when config('two_factor.enabled', true) is false, before any mail or audit write. It also applies to --dry-run, so an operator running the steps out of order doesn't get a misleading summary. Added testRefusesToRunWhenTwoFactorIsDisabledGlobally (exit code 1, warning shown, no mail, no audit row, for both normal and dry-run).

@romanetar This changes the decision recorded in doc/adrs/0001-recovery-code-generation-not-triggered-by-group-enforcement.md, which states that recovery codes for group-enforced users stay on-demand. The ADR isn't updated in this PR.
Suggested fix: mark ADR 0001 as superseded, or add an ADR 0002 describing what the command does (whatever shape survives the comment on line 101). Then the repo's decision log matches the behavior. The SDS §3.6 amendment you mention in the description belongs in the vault and is tracked separately.
After the change on line 101 the command no longer generates recovery codes, so ADR 0001 is not superseded: recovery codes for group-enforced users stay on-demand (profile "Enable" / "Regenerate Codes"). I added doc/adrs/0002-enforce-admin-2fa-command-notifies-without-generating-codes.md describing what the command does now: notification only, skip keyed on the enrollment_changed audit row (source=enforce-admin-2fa) written after the mail, and the refusal to run when two_factor.enabled is off. It links to 0001 as complementary. The SDS §3.6 amendment stays out of this PR as you said.

🟡 Changes recommended
A recovery-code generation race can invalidate users’ newly saved codes, while unescaped email names allow HTML injection and retained Doctrine entities risk memory exhaustion.
Adds idp:enforce-admin-2fa to onboard users in configured MFA-enforced groups while preserving group-based enforcement rather than changing stored enrollment flags.
Changes:
Processes users in 100-ID chunks, generating recovery codes for users without unused codes and recording enrollment audit events.
Sends synchronous notifications without recovery codes, with dry-run support and processed/skipped/error reporting.
Adds unit and integration coverage for processing, skipping, dry runs, unchanged flags, and error handling.

Becomes moot with the amended SDS (rev 2.5 §3.6): the command no longer generates recovery codes, see the comment on this line. Will be resolved with that change.
The reason will be displayed to describe this comment to others. Learn more.
Covered in 59083ee: the command no longer generates recovery codes, so there is no generateRecoveryCodes() / deleteAllForUser() call that could invalidate a batch the user saved in between. The race and the atomic generate-if-missing operation are not needed anymore. Users get their codes from the profile (Enable / Regenerate), as recorded in ADR 0001/0002.

Valid. Impact is low (the enforced groups are four admin groups), but the inline comment overstates it. EntityManager::clear() after each chunk is a cheap fix.
EntityManager::clear() now runs at the end of every chunk, after its mails were sent and audit rows committed (also in dry-run), and the comment on the pagination reflects it. The loop only carries $last_id (an int) between chunks, so nothing depends on detached entities. Added testProcessesMoreThanOneChunkAndClearsTheEntityManager: 101 users (> ChunkSize), asserts 101 mails / 101 audit rows and that the unit of work is empty at the end. I checked it fails without the clear() (345 managed entities) and passes with it.

Escape the output of user-controlled values.
{!! $user_fullname !!} on Line 7 prints the name without HTML escaping. The name comes from User::getFullName(), which joins first_name and last_name. Both fields are user-editable. A name that contains markup is injected into the email HTML. Use {{ }} for the name and for profile_url.

Valid, low impact (the name is user-controlled; the mail goes to the account owner). Switching to {{ $user_fullname }} / {{ $profile_url }} is a trivial fix.

@smarcet Agreed. The recipient scope limits the impact. Escaped Blade output is the appropriate fix for the name and for both the profile link’s href and displayed text.

Covered in 3401df0: $user_fullname and both occurrences of $profile_url (the href and the link text) now use {{ }}, as does the tenant name. TwoFactorEnforcedMailTest checks that a name with markup is rendered escaped.

@romanetar Per the amended SDS (ftn-docsnsklz sds/idp-mfa.md rev 2.5, §3.5 / §3.6), the command must not generate recovery codes. The plaintext returned here is discarded, so nobody can ever use this batch, and the profile still shows "10 of 10 remaining" with no low-code warning.
Suggested fix: remove the generateRecoveryCodes() call and the countUnusedRecoveryCodes() skip. Group-enforced users get their codes in the enrollment modal at their next 2FA challenge, or from the profile (§4.10.3, separate ticket). Update EnforceAdmin2FACommandIntegrationTest accordingly: no rows in user_recovery_codes, one audit row, one email.
The reason will be displayed to describe this comment to others. Learn more.
Done in 59083ee: generateRecoveryCodes() and the countUnusedRecoveryCodes() skip are gone (IRecoveryCodeService is no longer injected). EnforceAdmin2FACommandIntegrationTest now expects no rows in user_recovery_codes, one audit row and one email per user; the unit test was adapted too.
One thing to flag: today, group-enforced users can only get their codes from the profile ("Enable Two-Factor Authentication" generates the first batch, "Regenerate Codes" replaces it). I did not find an enrollment modal at the 2FA challenge in this branch (login.js only handles recovery-code verification and the low-codes warning, and ADR 0001 rejected that option), so that path would come with the separate ticket you mention (§4.10.3). ADR 0002 and the email copy only describe the profile path.
<p>Two-factor authentication (MFA) is now required on your account. You will be asked for a second verification step each time you sign in.</p>
<p>Recovery codes have been set up for your account. Recovery codes are shown only once, so to get a set you can save, sign in, open the Security section of your profile and regenerate them:</p>
The reason will be displayed to describe this comment to others. Learn more.
@romanetar "Recovery codes have been set up for your account" stops being true once the command no longer generates codes (SDS rev 2.5, §3.6). Suggested copy: recovery codes will be shown once at your next sign-in with the second factor, or you can generate them from the Security section of your profile.
The reason will be displayed to describe this comment to others. Learn more.
Done in 3401df0. The mail no longer says the codes were set up. New copy: "To set it up, sign in and open the Two-Factor Authentication section of your profile. When you enable it, a set of recovery codes is shown only once, so save it right away".
Two deliberate differences from the suggested copy: (1) I did not mention the codes being shown at the next sign-in with the second factor, because that enrollment modal does not exist in this branch and the mail would promise something users never see; it can be added with the separate ticket (§4.10.3). (2) I used the section's actual title in the profile, "Two-Factor Authentication", instead of "Security". TwoFactorEnforcedMailTest asserts the mail does not claim codes were already set up.
Onboards every user of the groups in two_factor.enforced_groups: generates
a recovery code batch, emails the user that MFA is now required (no codes in
the mail or the console output) and records an enrollment_changed audit event
with {"source": "enforce-admin-2fa"}.
Users that already have unused recovery codes are skipped so a saved batch is
never invalidated. Supports --dry-run, chunked queries and processed /
skipped / error counts.
It deliberately does not set two_factor_enabled / two_factor_method /
two_factor_enforced_at: enforcement is derived from group membership by
User::shouldRequire2FA().
…the mail
The command no longer generates recovery codes (users get them when they
enable 2FA from the profile). The skip criterion is now an
enrollment_changed audit event with source=enforce-admin-2fa, written only
after Mail::send succeeds, so a failed run is retried for exactly the
users whose email failed.
… is off
With the global kill-switch off shouldRequire2FA() is false for everyone,
so notifying admins that 2FA is required would be wrong. Return FAILURE
with a warning, also for --dry-run.
Records that the command only notifies (no recovery code generation, so
ADR 0001 stays in force), keys the skip on the audit row written after the
mail, and refuses to run when two_factor.enabled is off.
The full name was rendered with {!! !!}, allowing HTML injection into a
security notification. Also stop claiming that recovery codes were already
set up: the command no longer generates them, the user gets them when
enabling 2FA from the profile.
